If you cannot fill out these forms in advance, you may come 15 minutes early to your appointment to fill out paperwork.

A note:  If you are coming to the office and you find that the office door is locked, please call 704-200-3540 and I will come and let you inside.  For security reasons, the building is sometmes locked.   

Ballantyne Counseling is located on the third floor of the three story building at the corner of Ballantyne Commons and Rae Road, across from the Exxon station and Stonecrest.
